Register a death certificate

In order to transcribe a death occurred in the territorial jurisdiction of this Consulate General - Maine, Massachusetts, New Hampshire, Rhode Island amd Vermont, the following documents must be provided to this Office:

  • Certified copy of the death certificate full form;
  • Apostille from the Secretary of State to be attached to the certified copy of the death certificate;
  • Translation in Italian language of the death certificate;
    • If a translator is used, the translator's signature needs to be notarized and Apostilled; 
    • Otherwise please translate the certificate and forward it to statocivile.boston@esteri.it to make sure that it is correct;
  • Form to request the registration of the death certificate duly filled out and signed.
